The loudspeaker and earpiece are physically separate, in different parts of the chassis, with different grille designs. Liquid or debris rarely reaches both in equal measure. So when both sound identically wrong, the cause is usually upstream of the drivers — in software or the audio chip.
Play a mono voice memo and cover each grille in turn with a fingertip. Judging them in isolation is far more reliable than comparing while both play. If one is actually worse than the other, you have a physical problem in that one after all.
It happens after full submersion, where liquid reaches every opening. In that case both need clearing — but note they respond differently. The bottom speaker takes dry lint and responds well to cycles; the earpiece collects oily residue and responds better to putty and brushing.

No — and this matters. The sound has to travel through the blocked speaker to do anything, so any connected Bluetooth or wired device defeats the purpose entirely. Disconnect everything first.
