Never run cleaning tones into earbuds while wearing them. If you use tones at all, take them out, place them mesh-down on a cloth, and use around half the volume appropriate for a phone. The driver is a fraction of the size and the distance to your eardrum is millimetres.
Earwax, overwhelmingly. It migrates into the mesh with normal use and it is adhesive rather than loose — which is precisely the blockage type that acoustic cleaning cannot address. Skin oil and cosmetic residue add to it. Water is a comparatively rare cause.
Anything sharp punctures the mesh, and that mesh is the only barrier protecting a very small driver. Anything wet carries wax deeper and can reach the driver. Compressed air fired into the opening drives material past the mesh permanently. Alcohol attacks the adhesive holding the mesh in place.
Most earbuds have a second small opening that equalises pressure. Blocking it produces a plugged or pressurised feeling rather than muffled sound. If the symptom is discomfort rather than dullness, the vent is the likely culprit.
Try them on another device before concluding anything. A volume limiter, an equaliser or a stuck Bluetooth profile produces symptoms identical to a blocked earbud, and costs nothing to rule out.
If one earbud is quiet, that is usually wax. If one is completely silent, that is usually electrical — blockage reduces output but never eliminates it.

No. The frequencies used sit inside the range your speaker already reproduces whenever you play music, and the tool runs at moderate volume rather than maximum. The one genuine caution is earbuds: never run ejection tones into in-ear devices while you are wearing them.

No. Around 70 to 80 percent is the practical sweet spot. Maximum volume on a restricted diaphragm produces distortion rather than more displacement, and adds avoidable stress to the driver.
