A speaker grille has visible outer openings, a fine dust mesh behind them, and then the acoustic cavity containing the diaphragm. Debris behaves differently depending on which layer it has reached, and the layer determines which method can touch it.
Look with a light at an angle. Surface debris is visible as grey or fibrous material sitting in the openings. Hold a phone torch nearly parallel to the grille rather than pointing straight in — raking light reveals texture that direct light washes out.
Brush test. Thirty seconds with a soft dry brush. If the sound improves noticeably, the problem was largely on the surface. If nothing changes, it is deeper.
Onset test. Gradual degradation over months suggests slow surface accumulation. A sudden change after a specific event suggests something got in.
On a typical months-old blockage, most of the material is in the outer two layers and does respond. Brushing plus sweep cycles usually recovers most of the lost clarity. What remains is generally either bonded fibre or material that has passed the mesh, and both are outside what home methods reach.
The mesh is the only barrier protecting the diaphragm, and it is extremely thin. Puncturing it to reach debris behind it means every future bit of pocket lint goes straight through unimpeded. A partially blocked speaker is better than an unprotected one.

Yes. Water is held by surface tension and usually clears in one or two cycles. Dust is wedged mechanically and held by static, so it needs a sweeping range of frequencies and several more passes.
Thirty to sixty seconds per cycle, repeated two or three times with short pauses. Running it continuously for many minutes adds nothing once the accessible liquid is out, and only heats the voice coil.
For maintenance, roughly once a month is plenty. More often if you exercise heavily, work outdoors, or live somewhere humid or dusty. There is no benefit to daily use on a speaker that sounds fine.
No. The frequencies used sit inside the range your speaker already reproduces whenever you play music, and the tool runs at moderate volume rather than maximum. The one genuine caution is earbuds: never run ejection tones into in-ear devices while you are wearing them.
